Project Background:
The place-based collaborative clinical vision is for an integrated care model, embedded in strong, community-oriented and connected care systems, supporting individuals and families to live well, and achieve the heart of “what matters most” to them. A whole system Community of Practice (COP) was established by patient-facing staff to encompass and invest in developing cross-boundary relationships, promoting new situational learning and self-organising across the systems of health and social care in West Wales.
Same Day Emergency Care (SDEC) was the springboard to partnership working; it has been a conduit to connect staff and smash silo working, streamlining the patient journey, improving outcomes and experience. Building on the SDEC experience through the Value Based Health Care work, the teams joined forces with intermediate care, GP out of hours, 111 and social services in order to strengthen siloed working and intentionally communicate and co-ordinate care, pooling resources and colocating to form a central, clinical streaming hub “Porth Preseli.”
Project Objectives:
Transform clinical services in West Wales, through building relationships, the formation of a community of practice and a working co-operative in order to streamline processes into an integrated urgent care model.
Project Outcomes:
The establishment of Porth Preseli and delivery of an embryonic new Hospital@Home service and CATCH team. Diversification of staffing with the addition of a novel H@H pharmacy role and the introduction of advanced paramedic practitioners to the hub. Paradigm shift towards a proactive, preventative approach as opposed to the traditional reactive response in urgent and emergency care.
